ACL · The anterior cruciate ligament stabilizes the knee against pivoting and forward-sliding forces, and its rupture is one of the most studied and rehabilitation-intensive sports injuries.

ACL injuries range from partial sprains that retain some functional stability to complete ruptures, and are frequently accompanied by injury to nearby structures given the shared mechanism of trauma.

Common conditions

Severe

Complete ACL rupture

A full tear of the ligament, the most common presentation among significant ACL injuries, typically producing immediate instability and often a sensation or sound of a pop at the time of injury.

Moderate

Partial ACL tear

An incomplete tear that may retain some functional stability, with management decisions depending on the degree of remaining stability and activity demands.

Moderate

Concurrent meniscus tear

Damage to one or both menisci occurring at the same time as the ACL injury, common due to the shared rotational injury mechanism.

Concurrent collateral ligament injury

Injury to the medial or lateral collateral ligament alongside the ACL, more common in injuries involving a direct blow to the knee.

Mild–moderate

Bone bruising (contusion)

Microscopic injury to the bone beneath the joint surface, commonly seen on MRI after ACL injury and generally resolving over several months without specific treatment.

Moderate

Chronic ACL-deficient instability

Ongoing episodes of the knee giving way in someone who has not had reconstruction or adequate rehabilitation, which can increase the risk of further meniscus or cartilage damage over time.

Because associated injuries can significantly influence both surgical decision-making and rehabilitation pace, imaging and a detailed clinical examination are typically used together to map out the full extent of the injury.
