Biofilms ? Surface cities of microbes — plaque and catheters explain persistence, without a grow-your-own kit.

Biology here is spatial. A shaken flask is a different world from a coated catheter lumen.

Drug tolerance can come from slow growth and matrix, which is not the same as a heritable resistance gene.

Pieces leave. Downstream surfaces or tissues may be seeded.
